We know we do not get everything right all the time – and hence we welcome feedback, comments and complaints, as these all help us to appreciate how the recipients of our services feel about how we work.
INFORMAL
If you have a comment about your experience (good or bad) at the Forum Health Centre, then please let us know – either directly with the doctor, nurse or other health care professional – or with any of the receptionists.
You can also speak with the Reception Manager or Practice Manager – just ask at reception.
Any comments you make will be treated in confidence and will not appear on your medical notes nor affect you treatment and care as a patient.
We also welcome praise where it is deserved – it raises staff morale considerably when their efforts are appreciated.

FORMAL
If you wish, you can make a formal complaint, either in person or in writing, which will be handled in line with our complaints procedure.

You also have the option to refer the matter to the Parliamentary and Healthcare Ombudsman, although they try to ensure that local complaints procedures are exhausted before taking on a case.
